1. Urgent Phase
Objective:
ECC serves as your trusted guide, swiftly and effectively helping you navigate aging-specific urgent problems and questions. Our goal is to assist you in finding and establishing a good life for this present moment, tailored to your unique desires and context.
Scope:
- Housing and Safety
- Care and ADLs (Activities of Daily Life)
- Transportation
- Communication
- Medical Advocacy
Deliverables:
- Report outlining urgent items identified (digital and printed) and methodology for creating solutions.
- Post-report on how each urgent item was addressed (digital and printed).
Timeline:
This phase could take up to approximately 3 months after the assessment to complete. During our initial 500 point assessment, we will hear from you so that urgent priorities will be identified and addressed.
2. Planning Phase
Objective:
In the Planning Phase, ECC initiates essential future-focused conversations that you may have been avoiding. We believe that peace comes from preparation. By addressing uncomfortable topics now, we help you achieve peace of mind about the future. Everyone ages – the wise prepare ahead.
Scope:
- Legal and Estate Assessment
- Insurance and Benefit Analysis
- Strategy for Natural Decline
- End of Life Planning
- Long Term Preparation for all components of the Urgent Phase
Deliverables:
- Long-term report addressing each component in a 5-year plan with several contingency plans (printed and PDF)
Timeline:
This phase begins immediately after the Urgent Phase is completed and can take up to approximately 3 months to complete.
3. Routine Phase
Objective:
Between addressing urgent needs and planning for the future, maintaining a routine is essential. During this stage, Your Concierge conducts monthly check-ins to help you sustain the lifestyle and pathway established in the Urgent and Planning phases. Your ECC team will continue to communicate about future plans, remaining attentive about what lies ahead and making necessary adjustments.
Scope:
- Maintaining elements established in the Urgent and Planning Phases
- Assistance with coordinating Medical Advocacy
- Medication Management
- Technology Assistance
- Transportation
Deliverables:
- Monthly evaluation documenting proficiency of the current pathway, identifying potential adjustments, and executing as needed.
- Annual analysis and guidance for insurance selection.
- Annual Review of opinion scales from assessment.
- Annual Review of end of life checklist.
Timeline:
Some components of the Routine Phase will be established during the Urgent and Planning Phases. After the completion of those phases, additional routine coordination can take up to 1 month to establish, followed by routine monthly check-ins conducted by the concierge.
4. Legacy Phase
Objective:
Your story matters, and it doesn’t end here. We capture your legacy to ensure your life continues to guide and inspire future generations.
Deliverables:
- Multimedia Legacy Project capturing personalized vocal and handwritten messages to loved ones alongside of photography, milestones, and values-centered storytelling.
- The end result is a cherished gift perfect for memorial services and designed in a format to be utilized as a hallmark in a home setting for generations to come.
Timeline:
The Legacy Phase typically takes 1-2 months and can begin simultaneously with the Urgent Phase or as soon as you want to begin.
